What is a bladder stress test and a bonney test?

A bladder stress test is a test to stimulate unexpected urine output (urinary incontinence) that can occur when you cough, sneeze, laugh, or exercise.

The bonney test is done as part of a bladder stress test, after doctors prove that urine is excreted when coughing. This test is similar to a bladder stress test, except that the neck of the bladder is gently lifted with a finger or a device that is inserted into the vagina to put pressure on the bladder. This check is done to see if your incontinence is the result of the bladder neck pushing too far due to pressure.

When should I have the bladder stress test and bonney test?

A bladder stress test and Bonney test can be done as part of a physical exam when:

Your urine passes accidentally when you sneeze, laugh, cough, or exercise
Medical history, physical examination, and urine analysis did not reveal the cause of incontinence

What should I know before taking the bladder stress test and bonney test?

It is not uncommon for a woman to be able to withstand her urine (continent) while doing this test while lying down, but the risk of passing urine while standing can be triggered by the presence of gravity.

Both of these things may happen to a woman (stress incontinence and incontinence at the same time). The success of stress incontinence treatment can sometimes also help incontinence.

The Bonney test is difficult to benchmark. Therefore, the results are unreliable. It is important during the test to remove the tissue on both sides of the bladder neck and not just press on the bladder neck itself. If the leak stops when a finger or instrument is inserted into the vagina, it may be because the urethra is pinched shut, and not because your bladder is at the correct level. If the Bonney test is done to find out if surgery is necessary, the doctor performing the test must be very experienced to get reliable results. It may be embarrassing for some people to urinate while being observed.

What should I do before undergoing the bladder stress test and bonney test?

To help reduce the chances of developing an infection after the test, you should drink water, herbal teas and fruit teas, and juices, and stop drinking caffeine such as tea and coffee for 48 hours - to reduce bladder irritation.

How do the bladder stress test and bonney test work?

When you are lying down, a thin flexible tube called a catheter is inserted into your bladder through the urethra. You may feel burning when the catheter is inserted. If your bladder is full, it is not necessary to use a catheter.

Approximately 236.6 mL of fluid is introduced into the bladder through the catheter. When the catheter is removed, you are then asked to cough. The doctor looks at what fluids are lost and records the time interval between stress (coughing) and fluid loss. The stress test can be repeated while you are standing.

If the discharge is not detected during the bladder stress test or Bonney test, the test may be repeated while you are standing. Absorbent pads can be used to collect the urine that passes during your daily activities.

What should I do after undergoing the bladder stress test and bonney test?

You need to drink about 2.5 liters of fluid a day for 48 hours after the test. When you go to the toilet to pass urine, make sure that your bladder is completely emptied. You can do this by waiting a few seconds after you finish urinating and then trying again.

Explanation of the Test Results

What do my test results mean?

Stress incontinence is recommended if urine passes when you cough. People who lose fluids during a stress test may be helped by surgery that raises the neck of the bladder.

Slightly delayed fluid loss (occurring a few seconds after coughing) indicates that there is abnormal contraction of the bladder muscles. This is a sign of emergency incontinence. These contractions can be treated with medication.
