What is the dose of Piperacillin for adults?
Adult
IM / IV 3-4 g every 4-6 hours (maximum 24 g / day).
Serious infections (eg septicemia, nosocomial pneumonia, intra-abdominal, aerobic and anaerobic gynecologic, skin and soft tissue)
Adults and children over 12 years IV 12-18 g / day (200-300 mg / kg / day) in separate doses every 4-6 hours (maximum daily dose is 24 g / day).
Adults and children over 12 years
IV 8-16 g / day (125-200 mg / kg / day) in separate doses every 6-8 hours.
IM / IV 6-8 g / day (100-125 mg / kg / day) in separate doses every 6-12 hours.
IM 2 g as a one-time dose; give 1 g of probenecid 30 minutes before injection.
Prevention
Adults and children over 12 years
IV 2 g just before surgery, 2 g during surgery, and 2 g every 6 hours after surgery for not more than 24 hours.
IV 2 g just before surgery, 2 g 6 hours later, 2 g 12 hours after the first dose.
IV 2 g after pinching the umbilical cord, 2 g 4 hours later, 2 g 8 hours after the first dose.
IV 2 g just before surgery, 2 g back to the recovery room, 2 g 6 hours later.

Doses in pediatric patients less than 12 years of age have not been studied in adequate and well-controlled clinical trials.
