Function
What are the uses of Invokamet?
Invokamet is a combination of two oral antihyperglycemic drugs, namely canagliflozin and metformin. This drug is used to help control blood sugar for type two diabetes patients. Its use is balanced with diet and regular physical exercise to help control blood sugar more optimally. Invokamet is not used as a therapeutic treatment for people with type one diabetes and diabetic ketoacidosis.
These drugs work by restoring your body's natural response to the insulin it produces. Canagliflozin found in Invokamet helps the kidneys to remove glucose from the bloodstream through urine. Meanwhile metformin works by decreasing the production of glucose produced by the liver and decreasing the rate of sugar absorption by the intestines when digesting food.
Invokamet drinking rules
Invokamet is an oral medication that is usually taken twice a day at the same time as meals. Take this medicine in its entirety. Don't crush it, chew it, or break it into pieces.
Your doctor may order a series of kidney function tests before prescribing Invokamet. Your doctor can change the dose that has been given if needed. The dose given takes into account your body's response to medications, health conditions, and other medications you are taking. Do not change the dose or stop taking it before consulting your doctor.
Invokamet storage rules
Store Invokamet at room temperature, 15-30 degrees Celsius. Keep away from direct sunlight and humid places, such as the bathroom. This medicine must be kept in the bottle to protect it from humid temperatures. If this medicine is taken out of the bottle and stored in the daily medicine box, take it within 30 days. Keep this medicine out of reach of children.

The recommended dosage for adult patients with type two diabetes
Initial dosage:
- In patients who are not taking canagliflozin and metformin: use Invokamet which contains canagliflozin 50 mg and metformin 500 mg twice daily.
- In patients taking metformin: switch to Invokamet which contains 50 mg of canagliflozin and the same or close to the twice-daily dose of metformin
- In patients taking canagliflozin: switch to Invokamet which contains metformin 500 mg and the same daily dose of canagliflozin twice daily
- Patients taking canagliflozin-metformin can be changed to Invokamet at the same dose for both compositions twice a day.
Dosage adjustments are based on effectiveness and body tolerance. Gradually increasing the dose of metformin can reduce side effects on the digestive system.
Maximum daily dose: 300 mg canagliflozin and 2,000 mg metformin per day

Side effects
What side effects may occur due to consumption of Invokamet?
Some of the side effects that may occur due to consumption of Invokamet are nausea, vomiting, stomach cramps, diarrhea, flatulence, frequent urination, dry mouth, or feeling weak. If stomach problems return after taking the same dose of Invokamet after a few days or weeks, contact your doctor. These symptoms may occur early in treatment as a sign of lactic acidosis.
Some of the serious side effects that may occur due to this drug are:
- Pain, injury, infection in the leg that is at risk of amputation
- Kidney problems characterized by little urination, swelling of the legs, fatigue or shortness of breath
- Symptoms of a urinary tract infection include pain or burning when urinating, cloudy urine, and back pain
- Symptoms of genital area infection (of the penis or vagina), such as pain, burning, itching, redness, bad odor, and abnormal fluid production
- Using drugs containing canagliflozin can make your bones more brittle
This medication may rarely cause serious allergic reactions. However, if you notice an allergic reaction, such as itching, redness, swelling of the face, tongue, or throat, and difficulty breathing, contact your doctor immediately.

Warnings and Precautions
What should I know before using this medicine?
- Tell your doctor about all medicines you take, both prescription and non-prescription drugs, including herbal medicines. Some drugs can cause interactions if they are taken together
- Tell your doctor about any drug allergies you have, including allergies to canagliflozin and metformin. Also inform if you have any other drug allergies or other allergies. This medicine may contain other ingredients that can cause allergies
- If you plan to have radiological examinations that require injection of contrast fluid into the body, stop using this medicine. Contrast fluid is a substance that is inserted into the body in the process of radiological examinations (X-rays, MRI, or CT Scan) to see the condition of blood vessels in the body.
- Inform your doctor about any medical history you have, such as past or current illnesses before taking Invokamet to your doctor, especially if you have experienced severe kidney disease.
- The use of this drug may trigger lactic acid buildup in the body. This risk is increased in those who have certain health conditions, serious infections, are alcoholics, or are over 65 years of age.
- Metformin can also increase ovulation in premenopausal women, which can lead to unplanned pregnancies.
- Your doctor may prescribe additional vitamin B12. Only take as prescribed.
Is Invokamet safe for pregnant women?
The canagliflozin content in Invokamet can pose a risk to the fetus if it is consumed during the second or third trimester. Talk to your doctor if you are planning or are pregnant before taking Invokamet. Nursing mothers are also advised not to give their milk while taking this medicine.
Interaction
What medicines may interact with Invokamet?
The use of certain drugs at the same time may not be recommended because it will prevent one of the drugs from working properly. Even so, with a dose adjustment, several of these drugs can be prescribed simultaneously. Your doctor may make a dose adjustment if this is needed.
The following is a list of drugs that interact with Invokamet:
- Rifamycins (rifampin, rifabutin)
- Medicines for seizures, such as phenobarbital, phenytoin
- Ritonavir
- Contrast fluid
- Diuretic
- Insulin or other diabetes medication

Overdose
What should I do if I overdose on Invokamet?
In an emergency or overdose, immediately contact emergency medical assistance (119) or immediately to the nearest hospital. Symptoms of an overdose of Invokamet can include hypoglycemia characterized by weakness, nausea, trembling, loss of consciousness, or difficulty breathing. Dialysis can also be useful as a help to eliminate metformin in patients who are suspected of overdose of Invokamet.
What if I forget to take medicine?
If you miss your scheduled medication, take it as soon as you remember. If it is too close to the schedule for taking the next medication, ignore the missed schedule and continue on the normal schedule. Do not double your dose on a single medication schedule.
